IUI TREATMENT

Intrauterine Insemination (IUI) is a commonly recommended fertility treatment that helps couples facing difficulties in conceiving naturally. It is a simple, safe, and cost-effective procedure in which specially prepared and concentrated sperms are directly placed into the woman’s uterus using a cannula at the time of ovulation, hence increasing the chances of fertilization.

When IUI is recommended ?

So timing of doing the IUI is very crucial for maximum success rates . IUI is often advised in cases of unexplained infertility, mild male factor infertility, ovulation disorders, mild endometriosis, resistant PCOS, cervical mucus issues, coital difficulty, single woman or for couples who have been trying to conceive for a long time without success. There are some prerequisites like- fallopian tubes should be open and healthy, total motile sperm count > 5 million and no pelvic infections.

Steps of IUI

STEP 1 – Ovulation Induction and Follicular Monitoring

The process begins with monitoring the woman’s ovulation cycle which is often supported with medications to stimulate egg production (Ovarian Stimulation). Target is to achieve development of 1 to 3 mature eggs in a cycle to improve the conception rates. Serial transvaginal ultrasounds are done to monitor growth of developing ovarian follicles (Dominant follicle) and uterine lining (Endometrium)

STEP 2 – Ovulation trigger and timing of IUI

Once the ovulation window is identified by ultrasound assessment of a mature dominant follicle > 20mm and good endometrium lining of > 8 mm, a trigger injection (HCG) is administered to woman which felicitate the release of a mature oocyte/egg from the dominant follicle, it usually occurs at 36 to 40 hours from the time of trigger injection. Hence IUI is timed accordingly.

STEP 3- Insemination of washed Sperms into uterus

The semen sample collected by husband is processed in laboratory to select the healthiest and most motile sperm. Semen wash procedure is done using certain laboratory media, centrifugation and incubation techniques. This prepared semen sample (0.5 ml) is then inserted into the uterus through the cervix using a thin IUI cannula (access is through vaginal route using a speculum), making this process quick and relatively painless. This procedure can be done on an OPD basis without requiring anesthesia


IUI v/s IVF

Compared to more advanced treatments like IVF, IUI is less invasive and more affordable, making it a preferred first-line treatment for many couples. Point of difference between IUI and IVF is that fertilization happens inside the body of the woman (fallopian tubes) in IUI.

HOW SUCCESSFUL IS IUI ?

Overall success rate of IUI is around 10 to 25 % depending on several factors such as age, AMH, semen analysis, underlying fertility issues, and overall reproductive health.
